Dry eye is one of the most common reasons people come to see us, and one of the most misunderstood. It rarely feels the way you’d expect. In fact, one of the most common symptoms of dry eye is watery eyes.

Wait, watery eyes are dry eyes?

Often, yes. When the surface of your eye gets irritated and dry, it can trigger a reflex flood of tears, the same way your eyes water when you chop an onion. Those reflex tears are watery and drain away quickly, so they don’t fix the underlying problem. Your eyes end up both watery and dry at the same time.

Other symptoms include burning, stinging, a gritty or sandy feeling, redness, tired eyes, and blurry vision that clears when you blink.

The two root causes

Healthy tears aren’t just water. They’re a layered film with an oily top layer that keeps them from evaporating. Dry eye usually comes from a problem with one of two things:

  • Evaporative dry eye — the most common type. The oil glands in your eyelids (the meibomian glands) get blocked or underperform, so your tears evaporate too fast.
  • Aqueous-deficient dry eye — your eyes simply don’t produce enough tears in the first place.

Most people have a mix of both, which is exactly why a bottle of drops from the pharmacy often doesn’t solve it. Drops can soothe symptoms for a little while, but they don’t address blocked glands or chronic inflammation.

Why a proper work-up matters

Effective treatment starts with knowing which type of dry eye you have and what’s driving it. A dry eye work-up looks at the quality and quantity of your tears, images the oil glands to see how they’re functioning, and evaluates the surface of the eye. From there, we can build a plan that targets the cause.

What real relief looks like

Depending on your diagnosis, a treatment plan might include in-office therapies like OptiLight IPL to calm inflammation and restore gland function, gland expression, and lid-margin cleaning, along with simple at-home care and, when helpful, punctal plugs to preserve your natural tears.
